• Dental Screening (please print and return)

  • Students in Kindergarten must have a dental screening completed.

• Mandatory Lead Testing

  • Students in Kindergarten must have this test completed by their family doctor.

• Vision Screening (please print and return)

  • Any student in Kindergarten or 3rd Grade will need to have their vision screened.

• Free and Reduced Lunch Application (please print and return)

  • Any family who would like to apply for free and reduced meals must complete this form.

  • All 7th-12th grade students participating in school athletics must have this form completed by a medical professional.

• Concussion Form (please print and return)

  • All students participating in school athletics will need to fill out this form.

• Transportation Home From Extra-Curricular Activities (please print and return)

  • All student athletes will need this form filled out.

  • THIS FORM HAS TO BE SIGNED IN FRONT OF A NOTARY PUBLIC.

  • NOTE: You only have to file this form once. If you have previously filled out this form, you do not need to do it again. This form is good until your child graduates from High School.
